New group enrollment

Group plan requirements

  • An employer-employee relationship must exist.
  • At least 75% or more of eligible employees must enroll, except in groups of two to three where 100% must enroll.
  • To be eligible, employees must work 30 or more hours per week on a regular basis.
  • The employer must contribute a minimum of 50% of the employee portion of the premium.
  • The employer must submit the most recent quarterly tax and wage report.
