Washington supplemental benefits

For new or renewing groups with a January 1, 2014 to December 1, 2014 effective date, supplemental benefits can no longer be added as an extra benefit. All medical plans now offer a prescription drug benefit (outpatient). Some plans offer vision hardware, Expanded Choice and self-referred alternative care are not available on any 2014 plans.

Outpatient prescription drug—HMO

The Kaiser Permanente formulary applies to all plans. Members get up to a 30-day supply for each copayment (up to a 90-day supply of maintenance drugs for two copayments when our Mail-Delivery Pharmacy is used—except for the $15/50% plan). All of these plans, except the $15/50%, are Medicare Part D-creditable.

Choose one of the following options when purchasing an Added Choice plan. When a prescription is filled at MedImpact pharmacy, the plan covers up to a 30-day supply of generic drugs.

Self-referred chiropractic

Self-referred chiropractic care is available through approved Complementary Healthcare Plan (CHP) network providers in the Kaiser Foundation Health Plan of the Northwest service area. With the Added Choice plans, members may use their benefit at CHP, PPO, and other non-participating providers and facilities.

Alternative care

Self-referred care from Complementary Healthcare Plan (CHP) network providers in the Kaiser Permanente service area. Annual benefit maximum is $1,000. If added to Added Choice plans, members may use their benefit at CHP, PPO, and other non-participating providers and facilities.

Vision care
