Platinum plans

Summaries for platinum plans

Platinum plans offer the highest rates of all our plans and have the lowest out-of-pocket costs. Platinum plan types include copayment HMO and PPO plans.

Gold plans

Summaries for gold plans

Gold plans offer lower rates than platinum plans, and lower out-of-pocket costs than bronze and silver. Gold plan types include copayment HMO, deductible HMO, deductible HMO with HRA, and PPO plans.

Silver plans

Summaries for silver plans

Silver plans offer lower rates than gold and platinum plans, and lower out-of-pocket costs than bronze plans. Silver plan types include Deductible HMO, HSA-qualified high deductible HMO, and PPO plans.

Bronze plans

Bronze plans offer the lowest rates of all our plans with the highest out-of-pocket costs. Bronze plan types include deductible HMO, HSA-qualified high deductible HMO, and PPO plans.

Grandfathered (nonmetal) plans

These nonmetal plans were grandfathered under the Affordable Care Act on March 23, 2010. Groups currently offering grandfathered plans may continue to offer these plans along with metal plans, provided at least one employee must remain enrolled in the grandfathered plan for the plan to keep its grandfathered status.

Grandfathered (nonmetal) plan types include:

  • Copayment HMO
  • Deductible HMO
  • HSA-qualified deductible HMO
  • Deductible HMO with HRA
  • Point-of-service (POS)
  • PPO

Lower monthly payments, and doctor visits are not subject to the deductible.


 

HSA-Qualified Deductible HMO plans

Lower monthly payments, plus optional employee-owned savings accounts provide an innovative way to pay for qualified medical expenses.

An IRS-regulated, employer-sponsored program that allows your employees to receive tax-free dollars from you to pay for qualified medical expenses
